My 1994 saturn is running hot, changed the thermostat flushed the system?
but it wont overheat if I have the heat is on what could it be?

Air bubble in the system? it's to get caught when the heat valve is in the off position but when the heat is turned on the air bubble travels through the system, not impeding the flow of coolant to the radiator.
The flush and fill was intended to eliminate such problems if the mechanic followed procedures properly.
How long since the flush and fill? It should work itself out soon.

Have the computer sensors checked. You can also attempt using Castrol oil. I am not kidding, this really helped with a Buick Regal I used to have.

could be the fan switch, may be the water pump, may be the radiator is just to old, may be the transmission is over heating do to a blockage so over heating the engine, may be a blockage in the radiator system or air bubble, may be bad fan motor or poor performing fan motor.
first check to see if the fan is coming on, open hood, start the car wait and see if the fan comes on typically ten minutes or less.

they f'd up on the install of your thermostat, if you live were it doesn't snow, have it removed. most aftermarket thermostats are flawed, and because more stress on the water pump than common, if you must have a thermostat caused by snow or cold weather, investigate an open flow thermostat, it's costly but it guards the engine from over heating, if you didn't get the old thermostat in your hand or watch them put the new one in, then they may not changed it at all, thermostats are only good for three to five years at best, so have it totally removed if weather permits.

Apart from all the good answers, you may also check the fan relays for correct operation.
